Service Coordinator - Ebensburg and Johnstown, PA Improved Dwellings for Altoona, Inc.
is seeking a full-time Service Coordinator to join our team and split time between Ebensburg and Johnstown.
Basic Qualifications Education/Training:
Bachelor's degree in social work from a program accredited by the Council of Social Work Education, or a related field from an accredited college or university and one year of experience.
Skill(s):
Must possess basic knowledge and understanding of human behavior and motivation.
Must possess basic knowledge and skill in casework assessment and counseling.
Must possess basic knowledge and skill in group process and dynamics.
Must be able to express self clearly, both in writing and orally.
Must be able to relate to supervisor, clients, staff and general public in positive constructive manner.
Must be able to make decisions within the confines of established policies and procedures.
Experience:
A minimum of one (1) years' experience in a related position normally required.
General Responsibilities 1.
To be responsible for the supportive services coordination and overall coordination of supportive services at the development.
2.
To assess, monitor, and manage ongoing services needs of tenants; educate tenants on service availability, resources and referral mechanisms and provide case management services including intake and referral for all tenants needing such assistance.
3.
To establish relationships with area providers to ensure consistency and quality of service, provide advocacy supportive services and care management and provide necessary reporting documentation to IDA regarding units of service, costs, and other related activities.
Essential Duties 1.
Provides Case Management Services to residents, including crisis intervention.
1.
Develops or imports on-site programs, activities, and events for residents.
2.
Coordinates with site management on health, safety, and building maintenance issues.
3.
Works with property management to develop and administer rules and grievance procedures.
4.
Works with property management to create and develop outreach programs.
5.
Seeks out and utilizes supervision on an ongoing basis.
a.
Requests assistance from supervision on unusual and/or difficult cases.
b.
Participates in supervisory conferences and uses such time to review new and ongoing cases, discuss difficult and/or unusual cases, solve or resolve administrative and/or program concerns, and plan new or expanded group services.
6.
Participates in continuing education and in-service programs as a means to upgrade or add to existing knowledge and skills.
a.
Identifies programs and educational material of interest to self and other staff and shares with supervisor.
b.
Attends workshops, seminars, and conferences as approved by supervisor.
c.
Reports pertinent information back to supervisor.
d.
Reviews educational material provided by IDA.
7.
Conducts self in a professional manner at all times.
